Position Summary
In this role, you will be responsible for performing social services duties to meet the needs of both the residents and short-term patients admitted for rehabilitation services. The Social Services Coordinator also evaluates and plans for the resident population in accordance with current existing federal, state, and local standards as well as facility specific policies and procedures.
Essential Functions, Key Duties, & Responsibilities
- Assess residents, including mood, behavior and psychosocial issues, family relationships, family and community support systems, coping mechanisms, reactions to institutional placement, and assessment of level of care requirements to establish appropriate discharge plans for all residents.
- Work in collaboration with physicians and other healthcare personnel in resident evaluation and treatment.
- Document on charts, resident care forms, etc. on a timely and concise basis and ensure that all documentation is informative and descriptive.
- Complete the social history/assessment and discharge plans on each resident upon admission and provide regular updates as appropriate.
- Complete all needed assessments (MDS data, progress notes, social histories, and discharge plans) on each resident upon admission and provide regular updates as appropriate.
- Complete and present written and oral reports/recommendations concerning the social service department.
- Review departmental complaints and grievances from family, residents and staff and create written reports.
- Establish and maintain a good (communicable) working relationship with all other departments and within the community.
- Accept referrals and provide consultation with in-house staff regarding resident’s psychosocial needs.
- Interpret and communicate social, psychological, and emotional needs of the resident/family to the medical staff, attending physician and other resident care team members.
- Perform other duties as assigned.
- Comply with all policies and procedures and any updates.
Position Requirements
Education: Bachelor’s degree in Social Work or other related field
Licenses/Certifications: N/A
Experience: 2+ years in a long term care setting
