Position Summary

 

The Community Liaison is responsible for strengthening relationships with prospects, family members, influencers and referral sources via education, offering solutions, gaining trust and serving as a trusted resource. This position plays a vital role in qualifying prospects from clinical, situational, and financial perspectives and strategically applying appropriate tactics to proactively manage and convert prospects to admissions/move-ins. This role operates within a dynamic, challenging, competitive and changing healthcare landscape which demands a high level of creativity, focus, communication skills, speed and the ability to overcome objections and obstacles. The Community Liaison is responsible for actively promoting the organization in order to maintain a positive reputation within the medical community and the general community, while actively seeking out and identifying new programs and services to meet the changing needs within the marketplace. The Community Liaison is responsible for reaching and sustaining targeted SNF occupancy goals and corresponding activities to achieve said goals. 

 

Essential Functions, Key Duties, & Responsibilities

 

  • Identify, develop, strengthen and maintain referral sources to facilitate and drive qualified SNF referrals to the health care center, including post-acute, rehabilitation, long-term care and  
  • Build relationships with case managers, discharge planners, social workers, key clinicians, surgery schedulers, practice managers, physicians, specialists and key community influencers to grow census primarily within the SNF, but also throughout the campus. Maintain a consistent visible presence within these locations to increase the likelihood of immediate referrals and top of mind awareness. 
  • Develop an in-depth understanding of the campus programs and services to accurately highlight the clinical capabilities during discussions with referral sources. 
  • Build relationships in collaboration with other members of the team including but not limited to key influencers/decision makers representing senior-focused entities (centers, agencies and collaborations). 
  • Serve as a member of the transition of care team as it relates to vulnerable patients that are at risk for readmission or those that could benefit from home visits. 
  • Participate in weekly UR meetings providing feedback on sales activities and market conditions.  
  • Participate in weekly campus calls, weekly and/or monthly corporate calls to address census trends and actionable strategies. 
  • Participate in census strategy and update meetings. 
  • Attend select community-based networking meetings by maintaining an active and visible presence representing the campus. 
  • Maintain a pulse on the primary and secondary SNF competitors within the marketplace. 
  • Submit weekly sales call logs, sales pre-call planning guides, follow up, expense logs and other required reporting within the required time frames. 
  • Input required information within the CRM daily to ensure record keeping is accurate and up to date for all team members within 24 hours. 
  • Provide back up coverage for Director of Admissions. 
  • Provide sales/marketing support to other Franciscan Communities as needed or requested. 
  • Perform other duties as assigned. 
  • Comply with all policies and procedures and any updates. 

 

Position Requirements

Education: College degree preferred, with relevant experience in healthcare sales or admissions.   

Licenses/Certifications: N/A 

 

Experience:

  • Proven sales experience
  • Knowledge of healthcare marketing
  • Management of referral source relationships
  • Conducting market analysis
  • Proficiency in CRM systems
  • Development of business strategies

• Capacity to engage meaningfully with residents, potential residents, family members, and colleagues

• Insight into the target market and customer requirements

• Skill in identifying business opportunities, fostering client relationships, and effectively closing sales.

Travel: 80% of the time will be spent outside the community in hospitals, medical practices, skilled nursing and assisted living facilities, senior centers, and churches. Occasional travel to the home office for sales training or meetings may be required 1-2 times annually.
