Admissions Coordinator
Position Summary:
To facilitate the inquiry and admission process of nursing home placement for patients/residents (adults, ages 19-65 and geriatric, ages 66 and over) to this facility from physicians, hospitals and the general public, along with networking/promoting our services to the same.
Specific Task/Duties
1. Communications
- Communicates bed availability and discusses placement with hospitals and public.
- Tours families and caregivers through facility.
- Answers telephone inquiries from hospitals, referral agencies, families and caregivers including contacting potential residents in person and via phone.
- Increases referrals through networking with hospitals, physicians and facilities.
2. Admission Process
- Interviews residents/caregivers and obtains required information and signatures.
- Admits and prepares identification records for residents in accordance with established policies and procedures.
- Maintains an accurate record of available beds.
- Admits, transfers and discharges residents in accordance with established policies and procedures.
- Attaches pre-admission documentation to admission papers as appropriate.
- Collects, assembles and checks admission papers. Forwards to appropriate department.
- Completes PASSR paperwork if required.
- Determines eligibility of residents per Franciscan Ministries protocol including medicine, treatments, doctor office visits, psych issues, and other potential areas.
- Obtains the resident’s assigned room number. Places in all necessary admission papers.
- Explains to the resident and/or guardian, room rates, billing procedures, visitors/ guests privileges, restrictions, resident care procedures, etc., as appropriate.
- Notifies nursing service when the resident has arrived at the facility to assure that a member of the nursing staff escorts the resident to his/her assigned room.
- Maintains various registries as directed including register for admission and discharge of residents.
- Ensures resident room is clean and all areas including television are working properly.
- Maintains resident/patient and facility confidentiality.
- Obtains the resident/caregiver’s signature on all required permits, releases, authorizations, contracts, etc.
- Carries the Admission phone.
- Assists nurses with admissions whenever possible.
- Assists nurses and aides as necessary.
- Conducts on site clinical assessments.
3. General
- Develops and maintains a good working rapport with inter-department personnel, as well as other departments within the facility.
- Participates in staff meetings; keeps abreast of communications via posted memos and policies and procedures.
- Keeps abreast of current Medicare/Medicaid regulations governing admission/discharge requirements of health care facilities.
- Keeps work environment and patient areas neat and free of safety hazards.

Essential Functions
General Working Conditions:
- Subject to inside and outside environmental conditions.
- May be exposed to infection and occupational diseases, at times. May be exposed to internal/external disasters in which knowledge of appropriate policies, procedures and plans must be followed.
- Subject to frequent interruptions.

- Must be able to bend legs at knee to come to a rest on knee or knees.
- Must be capable of standing for sustained periods of time.
- Must be able to move about on foot to accomplish tasks.
