Social Service Worker

Full time - Exempt 

 

Position Summary

The Social Worker is responsible for supporting the emotional, psychosocial, and social well-being of residents within the community. This position provides counseling, crisis intervention, care coordination, advocacy, and resource referrals while working collaboratively with residents, families, healthcare providers, and community partners. The Social Worker plays a vital role in promoting resident dignity, independence, quality of life, and successful transitions throughout the continuum of care.

 

Essential Functions, Key Duties, & Responsibilities

  • Conduct psychosocial assessments to identify residents' emotional, social, and behavioral needs.
  • Develop individualized support plans in collaboration with residents, families, and interdisciplinary team members.
  • Provide short-term counseling, emotional support, and crisis intervention to residents and families.
  • Assist residents and families in adjusting to transitions, including move-in, changes in health status, hospitalization, rehabilitation, and end-of-life care.
  • Facilitate family meetings to address concerns, care planning, and communication.
  • Coordinate referrals to community resources, behavioral health providers, hospice agencies, support groups, and government assistance programs.
  • Advocate for resident rights, autonomy, and person-centered care.
  • Assist with discharge planning and transition coordination when appropriate.
  • Maintain accurate, timely, and confidential documentation in accordance with community policies and applicable regulations.
  • Participate in interdisciplinary care conferences and collaborate with nursing, rehabilitation, activities, dining, and other departments.
  • Monitor residents for changes in emotional or psychosocial status and communicate concerns to appropriate team members.
  • Provide education and support to residents, families, and staff regarding aging, dementia, grief, and available resources.
  • Facilitate resident support groups and educational programs as appropriate.
  • Assist residents with advance care planning discussions and provide information regarding advance directives when requested.
  • Help identify residents who may benefit from mental health services and coordinate appropriate referrals.
  • Promote a welcoming, respectful, and inclusive community environment.
  • Participate in quality improvement initiatives and maintain compliance with applicable federal, state, and local regulations.
  • Perform other duties as assigned.
  • Comply with all policies and procedures and any updates.

Position Requirements

Education: Bachelor’s Degree in Social Work required; Master’s degree in Social Work (MSW) preferred

Licenses/Certifications: Current unrestricted Licensed Clinical Social Worker (LCSW) license in the state of employment

Experience: 2+ years of clinical social work experience

Must have 2+ skilled nursing home experience

Skills & Abilities:

  • Excellent verbal and written communication skills
  • Exceptional interpersonal skills

Travel: Occasional travel required for community outreach or training events
