Assists in obtaining a DON/OBRA screen. - Submits appropriate Notices of Non-Coverage (ABNs) and Notice of Medicare Non-Coverage (NOMNC) to residents or their responsible parties. - Completes pre-admission assessment of prospective residents and readmissions utilizing the Senior Lifesteps assessment tool.   Required skills and qualifications: - Bachelor of Arts in Social Services or related Field. - Master’s Preferred - Minimum of two (2) years’ experience in a long term care setting.  - One year of supervisory experience. - Licensed Clinical Social Worker (LCSW) Knowledge of IDPH regulations. - Must have outstanding Critical thinking skills Outstanding customer service skills.  Addolorata Villa, is a proud part of Franciscan Ministries.
